1 . My 6 yrs of H1B get exhausted on 2008 as I was doing medical research( from 2001 to 2008 ). I am joining my medical residency on a J1 visa in 2008 which will last will 2011.

Am I eligible for a j1 waiver after 2011 ( inspite of having exhausted 6 yrs of H1B previously )

2 . If suppose I get a J1 waiver what "visa status" I would be while doing a waiver for 3 yrs in underserved area?

My understanding is that when one gets a J1 waiver the visa status during this waiver period is "H1B"

If I am wrong please correct me

You are correct. My understanding is that you will not be eligible for an H1b as you need to be outside of the US for 12 months to re-set your 'H clock'.

Your problem is not the waiver, your problem is the current requirement to be 'in h1b status' for those 3 years (so even if you got an O1 based on being a smart cookie, it wouldn't count towards your waiver time).

Consider looking for some way to work in canada for a year after you are done with residency. Provinces like BC have programs to get locums or permanent physicians to the northern reaches of the province. If you have US residency training, you can get a temporary license and a work visa without much hassle.
