Unsure if i should send this to KCC or my own country's US Embassy

I'm not sure if i should mail this checklist form to KCC or my country's US Embassy because i was told to keep this till my interview day. After i read it i feel suspicious, can anyone tell me what forms have you sent to KCC? If you can view the checklist form then i'd like to know if you have seen the similar form. Thanks!!
