Could somebody please advise whether or not I have found the correct form of UK police certificate required for DV 2008 applications?

Yes, that is the correct form. It took about 30 days from when we posted to receiving the report back from the police.

How important is it to include all my addresses for the past 10 years in the application form? I don't remember all of them

I was told every address you have resided in, no matter how short has to be listed.

i actually listed only uk-based addresses as they only check uk records. i have been here for less than 1o yrs
