Two files? Passed civics exam but clerk needs "whole file". WTF?

I'm helping a friend, this isn't my turf.

She, a 10+ green card holder just passed her civics exam, but they wouldn't authorize it or promote her to the next stage because they need her "whole file". She has never requested a replacement green card so she shouldn't have two numbers and her immigration lawyer is stumped.

Anybody else hear of this? Or know who she can talk to to get it straightened out?
 
Looks like they did not had her A file. That happens to someone here who did his interview in Newark. Please look for this thread.
