I am an Indian physician on J1 visa that has expired in July 2010. I still have the valid DS2019. I wish to attend an international medical conference in Canada. Can I do that without getting the US Visa renewed there.

Have you heard anyone trevelling on a DS 2019 without a J1 in their passport when they entered US first time?
DS 2019 is needed with a valid J1. You can't use one without the other. You should get a renewed J1 while you are there.

Yes! you can re-enter the U.S from Canada with your expired visa. The post here will not allow me to post the link for you but Google U.S. Immigration and Customs Enforcement website. Check the FAQ for F-Nonimmigrants: Entry and Exit (2E). Clearly explain all you need.
