Was I 'out of status'?

I plan on travelling out of the country soon but am a bit hesitant due to certain reasons- am listing it here, as briefly as I can. Would appreciate any feedback. Will I potentially be barred from re-entering?

Married in Jan 05.
I-94 expired end of March.
Applied for extension of stay before it expired.
Filed for GC early May.
Got rejection for 'extension of stay'- (due to incorrect usage of form) the day after I filed for GC - so didn't just ignored it.
Got AP in July.
EAD- approved last month but card has mysteriously not reached me yet.

Technically speaking, have I been out of status for any time?
If I travel, do I need to take proof of my 'extension of stay' documents with me to prove that atleast I had attempted to make my stay legal?

Just go there is no problem with your situation. I have travel with AP several times and the officers just look at it, sometimes make a copy, and welcome you! Almost no questions ask. Onetime, they asked me what was my travel reason. That's it. Ports of entry: Detroit, Port Huron, Chicago, California.
