translation issue !!

Hi,
My birth certificate is in Hindi. As per the application:
C. Translations. Foreign documents must be translated into English and "certified" by a competent translator (see format below)

CERTIFICATION: I, (insert name), hereby certify that I am fluent in the English and the languages and that the above is an accurate translation of the attached document issued by .

Date:
Signature, name and address of translator

I have mine translated & attested by a Notary from India. Is it mandatory to have it in the format above?
What is the quickest way to get this done? Is there any place in the US where I can get this done?

You can do it 10 minutes in US

Just translate your birth certificate to English. Then get any of your friend (should not be a relative) sign that. It is not necessary that he should be a notary. I have done the same. It worked fine for me.

You are right it's simplest way to translate into in English after saw this discussion I searched about this and I found very positive view about this.
