Transfer from H4 to F1

Hi People,

My wife is staying with me on a H4 visa. She intends to join school and move to F1 status. I have a few questions regarding this,

a> Will she have to go to India or she can take an appointment in mexico or canada and get her visa stamped.

b> If her F-1 is rejected for somewhat reason, will her H4 remain valid or will she have to go back and get her H4 stamped again. Would the situation be any different if her F1 is rejected in Mexico and Canada v/s India.

c> Can she just get her visa status transfered to a F-1 visa from H4 without going outside the country. I remember that I had changed my status from a F1 to H1 and had to get my passport stamped only when I was going out of the country. Does the same hold true in this case?

The visa stamping is required only if she travels outside of US and wants to enter back. Otherwise H4 to F1 approval does not require a visa stamp in passport.
If F1 is rejected, H4 is still valid.
