To change name on the birth certificate

Hi,

My name as per the birth certificate and educational records are different.I require to submit a copy of my birth certificate for green card processing.How do i go about changing my name in the birth certificate.What are the documents i need to procure or do i require any kind of affidavit.Please advise as what is to be done in the above case.

I had the same issue and my parents submitted a affidavit. I got new birth certificate from muncipal corporation after submitting the affidavit.
