Surrender application required 2 copies?

I am preparing to send surrender application for myself and family to housotn cgi, do I need to send two copies? photographs required for it?
 
they unclearly state that you have to send the application in duplicate, actually what you mentioned is exactly what they mean. u need the additional set of photographs for the copy just like the original set. just take a photocopy of the application (completed) and sign it.


i just wonder, when they charge $175.00 cant they spare the cost of taking a photocopy.

I went to get my SC in person to the CGNY, they asked me about the second set and had me go over a couple of blocks to get a photocopy. i asked them if they couldnt take a copy themselves, they said thats not their duty and its "clearly" mentioned in the directions!

I don't see anywhere in instruction to stick the photograph on houston form. Do we really need to stick photograph?
They don't have single form for all their embassy, isn't it stupidity?

I am not surprised with your experience in embassy, I would be surprise if they can help anyone to make extra copy or anything else. Lots people posted their experience and we know they won't improve. I am just praying they face the same problems (or more) when they have to deal with other government agencies:mad: When I read the news about Meera Shankar patted down at Jackson airport, I was happy and thinking more Indian officials should face these problems to understand our pain:(

i didnt have the photos pasted on the form. i just handed it to them. but against all odds the i got it the same day.
 
In NY, the CGINY does not require photos for the surrender certificate/passport cancellation process. The form does not ask for photos to be pasted anywhere. At least this was the case a few months ago when I got my surrender certificate and passport cancellation done.
 
then it might have been for the " complete the form in duplicate for Entry visa or for the OCI..
