Surname Issue

Hi All,

I have an issue with my surname. In my SSLC (10th) marks card I have my name as Sudeendra R.

In my Passport I have my name as Sudeendra Ramesh.

Ramesh is my Father;s name.

But eventually I started using my surname as "Rao" which is my family name.
All my documents PAN Card/Bank statement/Company records point to Sudeendra Rao.

Please let me know if this is a big issue and also please suggest me the remedies. I have an B1 appointment coming up next month.

You need documents to show that all three are the same. And on DS 156 fill in all names that have been used.

Get a notarized document indicating that all three names refer to the same person.
