Supplementary Form to I-693

Need help...
I just got a notice from INS VSC saying that the I-693 I presented when handing the I-485 on 8/99 was not accompanied by the Supplementary Form
to I-693. What does it mean? I checked the INS website and did not find
any related form.
My status:
ND 8/18/99
