Suggestions needed ...... Help required

I applied and got my initial assessment in Nov, 2002. In the application form, I filled my wife's last name (surname), although there was no last name in her passport.

In the medical form we received, they put her first name in the last name coulmn and last name in the first name coulmn ( I guess they consider the only name in the passport as her last name).

Now we also made a new passport having both of her first and last name.

What are the options here:

1. Can I just leave the things as it is as the name on the medical forms are for identification purpose and complete our medicals as required.

or,

2. I should let CIC folks know about the new passport so that they can made necessary correction at this point.

or

3. Considering this is not a big issue, Should I wait until CIC ask for our passport for stamping, and then they will made the necessary corrections by looking at the passport.

4. I also made my new passport as it is expiring soon. Do i need to give this information to them or wait until they ask for it for the stamping?

5. Will they use our names on the visa stamp or it is just a generalized immigration stamp for everyone.

Any inputs will be highly appreciated....

Before you go for medical, get the name changed. My wife had the same problem. It took me 3 months to get this mess straightened after the medicals were done.

or,

Yes, you will have to fax copies of your new passport

When I migrated to Canada, they were not stamping the passport. so someone else can answer this.

Any inputs will be highly appreciated....

How should I approach this name change. Just faxing a covering letter with the new passport phtocopies to CIC buffalo will be enough.

You think I need to inform Health Program (RNH), Ottawa to recity the name change on the medical forms.

Please suggest....
 
Hi Jalma,

You have to fax it to Buffalo at the same time you can
also mail a copy of the Passport and a brief covering note.

Better to be safe.
