Suggestion needed: E-Filling EAD/AP(Renewal)

I am currently using H1B Visa. If the 7th yr extension goes good then may wish to continue H1B.

Questions on E-filing EAD:
* I have both A# (current EAD) & I-94#(Last Entry using H1B) there is an “or” option for these. Which should be opted?
* I donot know the notice of Acceptance of my last EAD, But know the date my lawyer used on last year Application form, will that work?
* What all is expected to provide in the section "(c)(9) Please provide information concerning your eligibility status"?

Question on E-filling AP:
*Part 7: Requesting a letter explaining the reason to grant you AP. What should be expressed here?
