Applied for H1b Extention and it got approved after 6 months, My wife applied for H4 extention, there was a query in december 02, we answered this query, Online status shows another query was issued on Mrch 03, but my attorney did not receive any letter from INS. She called INS, to find out whats happening, the IIO said she sees no query on the file. Is any one with similar issue.

No need to go for H4 extension, you may get the H4 stamped on your return journey from your home country, based on your H1B approval, prior to her I-94 expiry.
If you do not have plan to go overseas then may be you need to get the extension to remain in status, not sure !

her I-94 expired, but she has valid EAD. To be on safe side I filed H4 extension also and the same goes with me. But, My H1 is approved but she got query (only on online status) but when called IIO by my attorney they see no query on her file. [bit worried, for the inconsistency]

If your wife is having EAD why do you worry about H4. If you want to go to India apply for AP. You should be ok. By the way when did you apply for the I-485.
As long as you have a valid H1, you and your wife will be in the safe status. If you want to get H4 Stamped why don't you send to Department (in Washington) to get H4 Stamped. See Immigrationprotal.com or Immihelp.com for Spouse Status stamping.
