Sticking papers together. DS-230 forms for 4 people and how to attach them correctly

We have DS-230 forms for 4 people (my mom, my older sister, my little sister, I am)

- It is okay to fill forms with black ink?
- There's some A4 papers where name of those whom about this form isn't written, so how better to stick them together? I know that I should put case number and the top right hand corner of each paper, but can I put the names of people for whom I fill this papers?
 
The question is - if in envelope we put 4 papers of DS-230 Part II (Page 4 of 4), for 4 person in our family, how they will determine whom each paper belongs? Therefore - should we put the name of applicant near to case number on each sheet?
 
There are staples that can be used in order to put a form together. Believe me its not the most complicated thing that you will have to do.

This question about forms is not the first asked. I already started more than 50 topics with different small questions like this one, so everything already highly complicated.

Do I really can use staples? I heard that it is forbidden to include in parcels any metal stuff.
