STEM OPT Denied – SEVIS History Issue, H-1B Consular Approved

My STEM OPT was denied on Sep 11, 2025. The denial notice stated that I was not on active F-1/OPT status and that USCIS was unable to pull my SEVIS history.
Background:

• My Initial OPT expired on Jun 27, 2025.
• My DSO confirmed I was active in SEVIS until Sep 12, 2025, but there was a system flag earlier (Nov 2024) when my record was terminated/reactivated by the time my H1B approved.
• I do have an H-1B approved (I-797B – consular, no I-94) from Nov 2024 to Sep 2027.
  
  I’m trying to figure out my options now
  
  Would appreciate any guidance.
