Statutory Declaration of Common-Law union form question

Hello, I have been requested by Canadian immigration to complete a Statutory Declaration of Common-Law union to add my spouse to my skilled worker application for immigration to Canada, since I recently got married.
Questions:
1) Based on my research a statutory declaration of common-law union is to show the Canadian immigration that I and my spouse have known each other for the past one year. Is this correct?
2) The form requires dates for the conjugal relationships. Do I put for the FROM date (date we got married) and the TO date as being current? Or do I put dates from when we started talking and getting to know each other? You all know how the Indian system work!
3) Will I need to provide any additional documentation with this form?

I would appreciate if someone could share their experience with this form.

This is what you have to do to add your spouse to your existing immigration application:

1) Second page of Application for Permanent Residence IMM 0008,
2) Updated Additional Family Information IMM 5406,
3) Your spouse's Schedule 1 and IMM 5406,
4) Your spouse's birth certificates,
5) Marriage certificate,
6) Spouse's education documents (You can get upto 5 additional points)
7) Photos
8) Application fee in the amount of CAD 550 for spouse (Please check)
9) Photocoy of her passport
10) Cover letter requesting them to add your spouse to your application.

As you are married I dont know why they asked you to complete a Statutory Declaration of Common-Law union :confused:

Hi Sprint,

I would fill up the above mentioned forms and arrange the documents mentioned as soon as possible and mail everything to the RPC handling your case.

In the cover letter you can mention that you got a request to complete a Statutory Declaration of Common-Law union (send the photocopy of the letter you received). Mention the fact that you are married and ask how to proceed further.
