Statewise variation in H1 requirements

Is it true that a physician license is not needed for a H1 if you are employed in certain states (eg.Wisconsin) where as you will need it if you are employed in other states (eg.Illinois)? Newyork doesnt need a license if you are a resident or a fellow and hence you dont need it for h1. But otherwise, why is this discrepancy between states when H1 is more of a national issue?
 
if ur in NY u get a license for fixed number of years on H one does need to have cleared all the exams and state license for getting H if it is for a job offer not for residency
