Start date of 6yrs period

Hi,

I was wondering about the start date of 6yrs for H-1 visa. Following are the facts of the case:

-Had the first H-1 visa stamped in 1999 (for Company A), but never came to USA on that Visa & got it cancelled subsequently in India.

-Got the second H-1 visa stamped in 2001(for Company B) and reached USA the very next month.

For counting the 6yrs period:
-Does it start from 1999 i.e the date of first Visa stamping OR
-Does it start from 2001 from the date the second Visa is stamped OR
-Does it start from the Date of Port of Entry in USA.
