Thanks.
  1. Approval Notice
  2. I94 (all copies)
  3. EAD
  4. AP
  5. 2 ADIT Photos (Be sure to get it done according to specs.)
  6. Passport

sai-2367,
I don't have I-94 forms (Got stolen), kind of wondering Why we require 94 forms. Since we are on EAD. I-94 is no longer required. Please correct me if I am wrong.

See if you have a copy of I-94. You would have got it along with your H1-B Visa approval. If you do have the copy take it, and explain the situation.

I think, they want one to surrender all duplicate artifacts for I-551, to avoid fraudulent use.

Sai,
We have to surrender the I94 when you leave the country. right?
then, how come we have the old I94? Everyone should have the latest I94 with the passport. I think that should be enough. Please, clarify it, friends....

Once you have I-551, you are supposed to use that for all travel purposes. They will not allow you to have duplicate artifacts for working or traveling. That is why they need you to surrender EAD's , AP's and I-94's.
In my case since i had two I-94's (both valid), since I had changed jobs before applying for I-485, I was asked to surrender both of them.

So the answer is this:- You might have to surrender any valid I-94's including the latest I-94. However the latest I-94 is what they look for.

sai, thanks for your information.
gc_san:
I94 is the arrival/departure card attached to the passport(white colour card).

No, I-94 that they want is normally attached to the I-797A(H1-B). It has the validity period on it.

I think you are refering to the white form that one fills on arrival and departure.
