Stagte 20 told me that they have not received any updates for past 5 yrs? is it bad?

State DOH told me that they have not received any updates for past 5 yrs? is it bad?

I called DOH, and they told me they have not received any updated from sponsor.
is this going to be problem for final submission?
does DOH give any confidential information to BCIS?

I don't have any experience in that. Someone gone through this would have better response.....

But here are some thoughts...

- Employer should be able to issue a letter stating that you have spend 5 years.... I know some have problem getting this.

- your W2s which should have the same salary as when you stated in your J1 waiver application. This would show that you were indeed, at least, paid by this employer.

- Daily record of patients you see. I know this might be a little difficult if you yourself are not keeping track. But billing summary should help. The idea is that if you can prove that you were seeing x patients per day at this office, then you were working at least 40hrs at this location.

There is a lesson to be learned for all of us that we should be aware of the fact that we might need this after 3 years. And we should plan from day 1 how we're going to approach this after 3 years are up.

Anyone know what lawyer(s) are saying what is needed and alternatives??

JCAHO/HIPPA confidentiality prohibits u to show patients records

you have to be careful about showing patients records to anyone including especially federal govt or INS. That itself is potential
other evidences: Rent (HPSA) receipts, telephone bills, other expenditure bills, any communications with DOH with records to vaccinations or general advise of patient care by medicaid or medicare or DOH. Of course W2 with pay stubs. and donot forget sign in and sign out sheet.
 
I did not mean to show patient records. I was referring to was how many patients you saw each day NOT the contents and who you saw.

Sign in/Out sheet is good idea.
