Spouse 485 and some questions about documents to be filed. please guide.

i am about to apply for my wife's 485, EAD, AP.

a) How many pictures am i supposed to submit for her 485, EAD and AP.

b) I heard that from now on, INS requires you to submit passport size photos instead of immigration photos? please guide.

3) I am supposed to take her to doctor to get a medical certifiate? if so, what kind of form i need for that? where can i find the doctor's list. i live in phoenix but my case was filed from chicago.

4) Am i supposed to submit affidavit of support (I-134). if so, where can i find 1-134.

5) Am i supposed to submit my emplyment letter, pay stubs, taxes and w-2 forms. i changed my job after 180 days ( got laid of) and found a new job in exact same category but never filed for AC21. i do not want to submit AC21 now as i already have obtained RFE ( last week) in which they have asked nothing abt my job status and have asked me to submit affidavit for birth certificate. please guide?

...It's good idea to file AC-21 in-case you have approved I-140 & Wait I-485 > 180 days.

...Just go through instructions and include all that is listed. You might want to add cover letter.

wife with 140 approval

I am 'adding' my wife on my 485 application and am filing for it myself. She is also pursuing her own GC application independently through her employer via the 140/CP route. Her 140 recently got approved and a NVC case has been created and we have sent in the fees.

My question is on the 485 application, in PArt 3.A, the last Question is 'have you applied for PR status before and what is the date and final disposition of the case?

In her case, the answer wouldbe 'yes' but which date do I use, her 140 approval date or NVC case create date? also, in her case she does not have an 'A' number yet.

Also, I spoke to my company lawyer about the requirements and he said I do NOT need to file I-134. I would apply thru him but his legal fees are $1500 which I would like to save
