1 - Should we type N/A or None even if the question does not apply to your specific case? e.g.
If you are a lawful permanent resident alien, complete the following:
My wife is a US citizen. Should we type anything in there? There are some other fields like this one. For example: Name(s) of Prior Husband(s)/Wive(s). I guess for this one we can type None?
2 - On my I-94 there is no leave/expiry date. It says F1 D/S. What should I write to the "Date authorized stay expired, or will expire, as
shown on Form I-94 or I-95" field. In my I-20 it says that the student is expected to complete studies not later than 02/21/2009. Should I use that date?
3 - Can I use abbreviations like Dominican Rep instead of Dominican Republic? The country field is too short in the PDF.
4 - "18. Address in the United States where your relative intends to live.". This field does not have any space for ZipCode. Where can I enter that information?
5 - "Your relative's address abroad.". Should I put my parents' address into this field?
