Social Security Card

Hi everyone,

I have a question about the social security card. My husband and I (I am DV2009 winner) we went to US in December 2008 in order to activate our green cards. We went to the Social Security office and gave them our filled in forms (which we downloaded from their web site). They said that if we have selected the checkbox 'SSN to be issued automatically' in the KCC forms then we have to wait for 4 weeks, so the social security cards will be sent to us. We waited for 3 weeks and we didn't receive our cards. Because we were planning to leave US we went back again to the Social Security office and told them the situation. The lady told us she will accept our forms but she will set the status on Hold till end of the 4th week and if no SSN has been issued then she will apply for us. Yesterday we received a letter from Social Security administration and it says: We can not give you a social security card at this moment because:
-You didn't give us the documents we need to show the identity
-You have not come into the office as we asked (we went even twice!)

can anyone in this forum tell me what we should have given to them that we didn't??
I thought for receiving SSN we could simply go to their office with the filled in forms and we would get our cards within a week! but that was not the case ..at least not for us!

I am currently out of US. I will go back again in June. I don't know if that would cause any problem.
