Service Request Or Enquiry ??

My 485 is pending for more than 6 months now. N/C got cleared in March. Would like to put in a status check request with TSC. What can I do i.e. should I put in a "Service Request" or "Status Enquiry Request"? I don't know the terminology, pardon me for that.

Also, Please let me know how to put in the request.
