Separated for a long time - what to write in application

My wife left me about 5 years ago and went back to her country, but we are still legally married. She was a permanent resident before she left, but that was 5 years ago. She got her permanent residency through my employment based application. Now I am applying for citizenship and I have no clue how to answer some of the questions on the N-400 form.

1. Question Part 8. B5 - Should I give her last known address (that we lived together) in the US or her address in India?
2. Question Part 8. E3 - Should I say she is a legal permanent resident or tey to explain in the "Other" field? I have no clue if she has been visiting the US to maintain residency.

I could say she is not a permanent resident and I could be wrong. She could be visiting the US in order to maintain her status. I have no clue. If I am wrong in describing her status, would they hold it against me?
 
You can always add an explanatory note when you are concerned that the standard answers are inaccurate and the officer could use some help in understanding the situation. As long as it works to your advantage, put a note *see attachment #X" and add this attachment between the current page and the next page.
 
1: Current address if known otherwise 'Unknown'
2: Other: Separated. Unknown Status. (Its better than saying 'None' and then finding out that she's been living in Kansas all this time and is also applying for naturalization)
LPR is not automatically lost just because you are out of the country. (even if it is for multiple years at a time)
