self-petition NIW: part 5 in I-140 question

I am doing self-petition for NIW. I have a question regarding I-140 form part 5: Since I am the petitioner, should I leave part 5 in I-140 form all blank even I have a permanent job now?
DO I need file ETA 750B? Some people so NO, some say YES. I am not sure.
Can some body advise his own experience here. Thanks

I got I-140 approval in NIW. I marked on the permanent job, because my lawer said if any job without any termination date may count as permanent job.

Yes, you must fill-up ETA760 partB.
