Self employment question

What address do I indicate if my wife is self employed and working from home?

We changed houses in the past 5 yrs, so do I put 2 separate entries with the 2 separate home addresses?

If your wife is working from home, then she must need to put her home address as her employment address as well. And must be ready to show tax-returns to the officer especially if she lives in NYC as officers always ask for tax-returns from those who are self-employed...
