selective service

Hi everybody,
Here is another question on collecting interview papers. I understand just to be safe I should have a letter from selective service showing that I did not need to register.
I became a permanent resident at the age of 29. Before that I was on H1B. So this should be straightforward. The form I am looking at is http://www.sss.gov/PDFs/SILForm.pdf . The question is what document did you send alongwith this ? I am thinking a copy of GC should suffice since it tells them my birthday and when I became a permanent resident. Or should I send a copy of every H1B approval notice also?

Your help is very much appreciated.

Thank you. My very first entry to US was on a tourist VISA (I went back after 10days or so). After that I came back on H1B and moved on to GC etc. So do you think I should send

-- copy of passport page showing tourist VISA
-- copy of passport page showing entry stamp for above
-- copy of passport page for first H1B
-- copy of passport page showing entry stamp for above
-- copy of passport page for second H1B
-- copy of passport page showing entry stamp for above
-- copy of front and back of green card

Does that include everything I need? Thanks a lot.
