Please tell me where you got the form for the Selective Service stuff.

Where the form was got from ? (post office, online pdf etc.,)

What is the form name ?

Did you send GC copy ?

Did you send PP copy or just the copy of the page with the first entry date ?

Was a cover leter written explaining your situation ?

Please help out

from the instruction page of the SSS.GOV link that you provided.

If you entered the United States as a valid non-immigrant alien, and remained in that status to your 26th birthday, you must provide documentation to support your claim. For example, if you entered the United States as an F-1 Student, and remained in that status until your 26th birthday, you would need to provide documentation indicating that you were admitted on an F-1 visa and attended school full-time as required. (Acceptable documents for this situation include copies of your I-20Ìs or a letter from the school you attended indicating your full time attendance as a non-immigrant alien).

For the above statement, I can show date of entry stamp on my expired passport, copy of my F1-VISA STAMP, copy of my first I-20 from FSC, then copy of my second I-20 from Tampa College.

The same thing applies for all non-immigrant statuses.

For this statement above, should I list all my; 4 H-1's that I have had or one line saying H1 from 10/1995 to 4/2002 ?

10/1995 to 09/1997 --- Company-A (copy of I-797 enclosed)
09/1997 to 06/1998 --- Company-B (copy of I-797 enclosed)
06/1998 to 03/1999 --- Company-C (copy of I-797 enclosed)
03/1999 to 04/2002 --- Company-D (copy of I-797 enclosed)

Also, dates should they be mm/dd/yyyy or just mm/yy ?
