SC fees requirements clarification

Hello All,
I am going to apply for the SC , I was checking the houston website, it says we need to send $20 fees, my question is does the fees covers the mail back charge also or we need to send the mail back charge separately in addition to the $20 , they did not mention about that.

Also do i need to send any other docs other than the original indian PP and the two copies of the renuication form?

any feedback or comments will be greatly appreciated.
