Sample letter for Emergency AP

I want to take an info pass appointment for emergency Advanced Parole (currently my AP is pending. Receipt date is Jan 21, 2011). I have to travel to india as soon a s possible because my father health condition is serious. Doctor who treating my father expecting a sample letter from me. So that he can review and send me letter in same or similar format. Pl. email me the sample letter, which you submitted to infopass, by deleting your personal info.
