Is there a sample cover letter to mail the I-485, AP, EAD, medical, Biometrics forms? What are the other documents that need to be mailed in this package? Thanks.

I-485 questions

For the A# in Part 1, do I mention the number given in the I-140 approval letter?
The other question in Part 3, Have you ever before applied for permanent resident status in the US?, Does this refer to I-140 application or any previous I-485 application. Thanks.

Yes, use the A# on your I-140. The question in part 3 refers to any previous I-485, not I-140.
