sample cover letter for AP

I am planning to apply for renewal of AP myself. Iwould like to know if
-anyone has a copy of sample cover letter to include with the documents
-Where to send the documents, the specific add.( I am under TSC)
-Do i need to send my spouses application along with mine or seperately?

Thanks for the post, but I need the cover letter not the support letter please.

Plus do I need to send the marraige certificate for renewal EAD or its necessary only the first time?

the cover letter == the support letter

It's better to send all documents even for second and third AP
