RTD for a minor

Hello everybody!

Does anybody know how is the process to get a RTD for a child (05 years old)? The child is a permanet resident now.

1. Do both parents need to sign the I-131 forms?
2. Does the child need to sign?
3. Does the mother or father sign for the child ("name of aplicant" by mother "name of mother")?

Any experiences?

1-No
2- No
3-Yes(only one Parent has to fill it out and sign) see on the application form, there is in the bottom a space for a person who prepared the application.
