RFP on medical for spouse. help

I received RFP on medical for spouse to go back to the doctor to compete the immunization part medical form. She did take all the required immunization last week, looking at the supplemental form the doctor have check marked the “ Applicant may be eligible for blanket Waiver as indicated above. When I asked the doctor why she did that she says she can not check mark vaccination history complete because she did not recive all the shots and she have to wait 1 to 3 month to complete the full vaccination.

Any idea not these matter.
