RFI - look like?

How does a RFI look like?
Does the letter supposse to have the IO# or something?
Bar code?
Does it came with a envelope to return the evidence?

USCIS sent me a RFI long time ago... but I never received, so I called them to resend it... but the letter I received is very simple and there is no return envelope, letter bar code, or IO#... and they even misspelled my last name...

Is this how it supposse to be? I am affraid that when I send the doc they will get lost or missplace....

The RFI is for my medicals and a new G-325A.

I got my RFI from TSC and it was the color and size of a NA. It did not have the IO# nor did it come with an envelope(though I hear those from NSC received a return envelope with it). Your docs will not get lost or misplaced if you enclose a copy of the RFI with your docs. And I recommend writing "Response to request for Intial Evidence for I-485" somewhere in the address on your envelope for it to find the department it needs to go to quickly.

As far I could recall my RFI (NSC) back in March 2005 had a IO's code number all the way bottom but no return envelope with that. But in RFE (NSC) Jan 2006 had a return envelope enclosed with it & also noticed the IO's code number different. Possibly IO in charge of my file was changed at some point on.
Yes, there is an IO code in both RFI/RFE. I never even noticed that unless Wantmygcnow our knowledgable moderator brought that to my attention at some point last year.

I did not notice IO# but then I did not look at every inch of the paper, so it might very well have been there and I missed it. But I am certain there was no returm envelope with my RFI.

I don't think for RFI anyone receives return envelopes but RFE I did as I said.
