hey this is what i got

For me:
RFE1: Supplemental Form To The 1-693: Submit the applicant's supplemental form showing his or her
immunization record. Please complete your immunizations, there is no national shortage of varicella.


For my wife:

RFE2: please provide an affidavit of your birth records. The certificate that you have submitted does not provide evidence that your maiden name is :Lastname, Firstname.

the certificate has "X.firstname" standard indian format.

Do we need to get medical exams again?
my wife is pregnent, she can't get those done now, how that is going to work?


Also for Date of birth affidavit how to get it? do we need to get it from her father / mother in india?

Isn't Varicella for chickenpox? Assuming it is - Doctor's generally ask you if you/your wife had chicken pox when she was young, if she did then
you are considered immune. But Im not sure if they take your word for it. Some doctors do, some donot.
Nowthat you already have an RFE on it, I would suggest, she take a blood test to check if she is immune or not. If yes, no need for vaccination. If not, just append a doc note saying she can not take vaccination because she is pregnant.

Yes. Ask her parents to provide two affidavits (one from her father, one from her mother). Be careful about the lastname, firstname. See the format in this site: http://www.immihelp.com/gc/aos/birth_affidavit.html . You can remove the clause #6 ("a request has been made....") as she has a birth certificate. If her lastname is changed after marriage, it better to mention both surnames - her maiden surname and surname after marriage.
