I am with the same employer for last 5 year.. any idea what it could be

RDMAR02, my RFE was for a tax return and W-2 for last year, paystubs for the last 6 months and an updated employer letter.

I have sent you the birth facts affidavit format, please check your e-mail.

AFFIDAVIT OF BIRTH FACTS

I, <parent’s name>, after being duly sworn, do hereby depose and say:

1) That I am a citizen and nationality of <country>, born on <parents birth date> at <parents birth place> and presently reside at <current address>.

2) That I have direct personal knowledge of the birth of <your name> as I am his <relation>.

3) That <your name> was born on <your birth date> at <your birth place>.

4) That his natural parents are <mother’s name>, mother and <father’s name>, father.

5) That this affidavit of birth facts is being offered, because <the birth certificate was just recently issued>/<the birth certificate does not have all the information>/<any other reason>.

6) That it is my prayer that this affidavit be accepted for this purpose.
