RFE is IN!...however I have a queston

Ok so I finally got that RFE in the mail...woohoo! and so far it sounds like they want my Medical Exam, However it gives me two things, this is what it says and please someone clearfy what their saying.

E. [x] (3) Submit a properly completed supplemental form to I-636, adjustment of status applicant's documentation of Immunization. The Supplement must be completed by a civil surgeon who has been designated by USCIS. to optain names and telephone numbers....etc etc please contact the Customer Service line.

All I-485 applicants must present documentation of having received vaccination against caccine preventable diseases, which shall include at least the following disease...(Lists all the diseases).

E. [x] (2) Submit a proerly completed and signed form I-693, Medical Examination of Aliens seeking adjustment of Status, for the applicant on form I-485. the Medical Examination must be conducted by a civil surgeon who has been designated by uscis. to optain names and telephone numbers..etc..etc contact the customer service line.

Ok so it sounds like they want my Medical Exam, the Package that the Doctor gave me and told me to not to mail it. However why did they repeat it twice? do they need another medical exam or something?

Also when I do send it, what should I include in my package, I under the Medical Exam but do I submit a cover letter or a copy of my I-485?

They need I-693 form and Supplement A to I-693 (basically list of vaccinations u received throughout years). Check with your doctor to make sure he enclosed Supplement A. My doctor failed to do it so we got RFE. When you collect it all, mail it to USCIS.

Well my Doctor gave me a copy of whats in the Envelop, and well I got the I-693 and Supplemental Form I-693, I also got X-Rays too.
 
Send it to USCIS with a copy of the letter requesting further evidence. Cover letter is always a good idea, to make their life easier.

Oh ok, should I make a copy of my I-485?

You should always make copies of ALL documents used in this process for self.

Send the RFE letter (make copy for self).


I don't know why the doctor would say don't mail the medical? Getting a RFE causes in most cases application process to be delayed.

Cover Letter should state what info you are providing along with RFE letter

Write on the outside of mailing envelope in big letters "Request for Evidence"

The doctor never said dont send in the medical....

so ill send in a Copy of my I-485, the RFE notice letter and Cover Letter.

Isn't that what you said in the above statement about the doctor gave me and told me to not to mail it :confused: ????
 
Not so long ago, you were required to bring your I-693 in a sealed envelope at the interview. Not anymore, but doctors are...doctors....not lawyers.
I wouldn't listen to them. And it's clearly stated on the I-485 instructions.
So your doctor was wrong and unfortunately you trusted him on that. :eek:
But no big deal, just a little delay.
