Hi Gurus,

I have given name in passport as Name-A Name-B which does not include family name, how ever my father name on the last page of the passport has family name.

I have been including my family name in my most of the forms here, my driver license and SSN have family name too. However my visa and I-94 does not.

I have an RFE on AP for documentary evidence showing these names are of same person.

All my I-485 biometrics have full name..

What documents could I provide ? Does anyone had similar issue ?

I have my expired Canadian PR card with full name, is it advisabel to use this ? What are the consequences of using Canadian PR card for GC ?

appreicate your help, thanks

Get a affidavit from your home country - stating that the names belong to one and the same person. Get it signed by couple of relatives, and I guess it will work.

In my case (No, RFE in my case) - I have used my First Name and Last Name throughout my life; Family name was never used, though the family is mentioned in the back of my passport along with my Father's name (just like your case)

I think they raised an issue because your passport has First Name and Last Name and all the other documentation has First Name, Family Name and Last Name.

As a precaution I do have an affidavit just for my name in case of an RFE. I think it is important to have a consistent name in all the documentation - i.e. starting from Birth certificate, School certificates, Degree, Passport etc.
