RFE for Medical report

I got the RFE for the following :

1. A copy of your Form W-2 for the last three years (2003, 2002 and 2001);

2. A copy of your three most recent paycheck stubs;

3. A copy of your Federal Tax Returns (signed by you and your spouse, if filed jointly) with all accompanying schedules for 2001, 2002 and 2003; and

4. A new Form I-693 (Medical Examination with Immunization Supplement) completed and signed by an authorized physician. The medical examination results on file apparently indicate that you have not met the appropriate immunizations requirements

My question is :
a) In item three what does it mean by schedules?
b) Have anyone got this kind of medical RFE?
 
Its great that things are moving for you. Please don't forget to post your RD/ND, RFE date, FP dates etc.

Regarding schedules, if you tax return is very simple... only W-2 income, no itemization, no child care credit etc. Probably you didn't have to attach any schedule to the 1040. Nonetheless, send a copy of all the pages of the tax return.

Regarding medical, visit any INS approved doctor and get the immunization done, get the form and sent to INS.

Yes. received medical RFE

Both me and my wife got RFE for medical one
after the other (for I-693 supplimental form)

Waiting after RFE has been received by BCIS on March 11th .....

Why Medical again?

Hi Ramakriya,

Did you take all medical tests again? Please give me the details. I am confused.
 
Depends on what your RFE is about.

For me, my INS chest X-ray report said there is a scar that may have to be evaluated. BCIS wanted clearance on that.

So I took another X-ray from my regular Doctor, took the report that there was no tuberculosis and then gave this to the INS certified doctor. He filled up the I-693 form based on that. (only one column was to be redone) ; Not a new test.

For my wife, she had been waived of the immunization record when we did the medical test, since she was nursing that time. BCIS wanted those details to be filled. She took a blood test with her regular doctor , and got her immunization record filled on that basis. The INS physician filled in the supplimental form based on this. No re-examination was done.

Your case seems to be like my wife's case too.
Go to your doctor ; Either take the required shots or get a blood test to prove you are immune to whatever is on the list (varicella-mmr etc) and then get teh supplimental form for I-693 filled by the INS physician
