Hi,

If we are asked to re submit medical records(I-485), do we have to do all that once again? (Like x-ray, immunization shots etc). Thanks

Is your I-693 supplemental not complete initially. From the board, I understand that you just need completed supplemental, not complete redo of shots and x-rays.

Thanks tsc-vsc. Even we are CSC-VSC. My husband\'s I-693 was complete. And mine is not because I was pregnant at that time. We have our supplememt I-693. Do they mean that? and the weirdest thing is I am NOT asked to bring I-693, but my husband is!(That too they have mentioned sealed)

We moved to VSC at wrong time, I guess. My wife was pregnant at that time and I am expecting RFE for her. Your case is weird. I took copies of supplemental from the doctor and my wife’s report says does not meet immunization requirements.

Did they return just your husband’s envelope. I would call IIO to make sure they meant right and to save time for another RFE for your medical. By the way, what are your RD/ND & EAC FP Date. Did the RFE also include pay stubs/letter.

Hi TSC-VSC,
I just spoke with IIO(NAtional customer service center, had to wait about half an hour). She told me we don\'t have to take the immunization shots again but has to do the other medical check ups again. But I still wonder whether I should do it or not. Because I don\'t know whether INS has forgotten to mark X for me.

They haven\'t returned any envelope.

The letter from them doesn\'t say RFE. It says this interview will be video taped. So I am hoping that it\'s approved.

I am not familiar with the dated. We originally filed for GC in June 95. We filed for I-485 in JUne 98(Beleive it or not!). Twice FP. Last one was in OCT 98.

Now the files moved from CSC to the local NJ office(Ofcourse we moved)

I don\'t know which category we are under. My husband has engg degree from India. No Masters.

I think one should call INS Service Center, not National Customer Service). VSC # is 802-527-4913. In your case, may be local INS office at Newark. You have been waiting longer than I have. Search from this board for previous messages on expediting the process by approaching Senator.

Hi,

Our case (me and my wife) was similar. One of the docs we were asked to
submit were properly filled out supplement to I-693 because it was not
filled out properly. INS had sent us the original I693 and the improperly
filled supplement ... basically, the physician had forgtten to put X marks in a
couple of locations. To be exact, this is what the form said:

        Your I693 was **not accompanied by a properly completed
        supplemental Form to I-693**, the determination cannot be
        made that you are current for all recommended age-specific
        immunizations.
        The Supplemental Form that ou submitted was not complete.
        At least one block in section 3 must be marked or a notation
        must be made for each vaccine in section 2. On your form
        there was no notation for 3 vaccines in section 2 and none of
        the blocks in section 3 were marked.

        Your original Form I-693 is being returned to you at this time.
        You are required to return to the physician who completed
        your medical examination to obtain a properly completed
        Supplemental Form to I-693. After the Supplemental form and
        the I-693 have been completed and signed by your physician,
        you must return both forms to this service. Please note that
        both forms must be returned to this service in an **unopened**
        envelope that has been sealed by your examining physican.

I called INS (VSC) and asked whether we should redo the whole thing or just
have the physician fix the old one, and IIO said it was up to us, so we only
had the physican (same guy) fix the old supplemental forms, and sign next to
the fixes he made, put the fixed versions in envelope, seal them, and hand them
to us. We sent them to VSC a couple of weeks ago, and the automatic message
says that our case has been approved, and that we should receive approval
notice in the mail within 2 weeks.

I have a simila situation where physician did not check the boxes under section 3 supplemental form to I-693. He had checked in section 2 , as waivers to be requested from INDS for certain immunizations. So should he check both boxes saying 1.Applicant may be eligible for blamket waiver(s) indicated above and 2.Vacciantion history complete for each vaccine , all requirements met. Or only the first box.
