Hi,
Our case (me and my wife) was similar. One of the docs we were asked to
submit were properly filled out supplement to I-693 because it was not
filled out properly. INS had sent us the original I693 and the improperly
filled supplement ... basically, the physician had forgtten to put X marks in a
couple of locations. To be exact, this is what the form said:
Your I693 was **not accompanied by a properly completed
supplemental Form to I-693**, the determination cannot be
made that you are current for all recommended age-specific
immunizations.
The Supplemental Form that ou submitted was not complete.
At least one block in section 3 must be marked or a notation
must be made for each vaccine in section 2. On your form
there was no notation for 3 vaccines in section 2 and none of
the blocks in section 3 were marked.
Your original Form I-693 is being returned to you at this time.
You are required to return to the physician who completed
your medical examination to obtain a properly completed
Supplemental Form to I-693. After the Supplemental form and
the I-693 have been completed and signed by your physician,
you must return both forms to this service. Please note that
both forms must be returned to this service in an **unopened**
envelope that has been sealed by your examining physican.
I called INS (VSC) and asked whether we should redo the whole thing or just
have the physician fix the old one, and IIO said it was up to us, so we only
had the physican (same guy) fix the old supplemental forms, and sign next to
the fixes he made, put the fixed versions in envelope, seal them, and hand them
to us. We sent them to VSC a couple of weeks ago, and the automatic message
says that our case has been approved, and that we should receive approval
notice in the mail within 2 weeks.
