Revalidated visa needs a correction, pls help

Hi everyone, I just got my revalidated visa from DC.
But unfortunately it has incorrect information, so I have to send it again. My question is what should I mark in DS-156, question 35 " has your US visa ever been cancelled or revoked" "Yes" or "No" if DC cancelled my previous visa because I've sent passport for revalidation 30 days before my last visa expiration.
Has anyone get same experience?

Cancelled or Revoked...... has got nothing to do with your situation.

Just say NO as the answer.

Anyway, I've decided to call to DC. The lady told me that I have to send only my passport, letter explaining the mistake and self addressed envelope. Nothing else. On my question, that www page require submitting the new application form, she said "no application form". Isn't it wonderful?

All the best.... This time they wont take more than 10days for correction.. as u waited already long.
