Revalidate H1

I had my H1 VISA revalidated in St.Louis in 2000,after that never went out of country.My old VISA is expired and I need to send my passport to St.Louis again to revalidate H1. My question is do I need to tear off the 1-94 form from old I-797 and attach to VISA page?Also do we need to attach one photgraph or two? Any inputs are welcome.

Donot tear the I-94 attached to the approval notice and just send one photo I just got mine back from St Loius Hope this helps

How long it took for you to get VISA revalidated?

excatly 6 weeks send my PP on May 30 and recd it on July 11 hope this helps
