Reval questions?? Help

Hi all,

I am about to send the papers for reval for my H1 and H4 for my wife, would appreciate if someone can answer some of the questions that I have

1) The total MO amount should be 100 + 100 + 50 + 50 = $300 right?, for I saw some earlier threads and they listed the amount as $350 for 2 passports.
2)Is a photocopy of the I 797 ok?
3)What do I need to send for my wife's stamp, also do I need to draft a seperate letter for her H4 stamp?
4)Apart from the employment letter, copy of I 797, completed forms and MO, do I need to send anything else?

Please advise, THANKS!!!!!!!!!!!!!!!
 
Do you have a separate Extension I 797 for your wife also .. Or you are sending your I-797 for her stamping also ???
