reschedule FP for parents

Hi

I have received the FP notice (ASC in SFO) for my parents . However, due to unavoidable circumstances we need to request for a reschedule date since they cannot be in SFO on the given date/time

Question

1) As per the FP notice, it seems to be a straight forward case of checking a checkbox and mailing it to the ASC involved. Do I need to write a letter or provide any documentation for this ?

2) Can they take the notice and go for a FP in anyother ASC center in the US ?
