Requirement for (prior) Indian passport

I am getting ready to apply for an OCI card at CGSF. Their website states that we need to submit a copy of the original Indian passport and also submit a copy of the surrender certificate.
I obtained US citizenship over 30 years ago and do not have my Indian passport or a copy of it. I do not have a surrender certificate either.
Any suggestions on how to proceed?

You may want to make an appointment with Consul at CGSF and explain to them of your situation. If you have proof of travel to India on Visa (visa stamp on your US passport), you may submit the copies of relevant pages with a affidavit, they may accept.

I got a surrender certificate at CGISF by submitting: police report (you can file it online in most Bay area cities for free); notarized affidavit; copies of old Indian visas from my US passport pages; I was lucky I had the PP number available though I did not have the date of issue exactly. The vice-consuls are very helpful if you submit all this and then go in person if needed (ie if they don't process it with just this above info).
