Required information on birth certificate

Dear CP gurus,
      My birth certificate is in my native language. It\'s a letter from gram panchayat as I was born in a village. Which information is required on a notarized english translation copy? My name, DOB, place of birth, father and mother name. Does it require any information other than i mentioned?

You should be doing fine with that.

You could also add that "the above info is an extract from the original birth records ".

Birth certificate translation

Translate your birth certificate line by line in English and then get noterised it by translator(not you) and get certificate for that in the following format:

                       Certification by Translator

I _______________________________________ , certify that I am fluent (conversant) in the English and ...... languages, and that the above/attached document is an accurate translation of the document attached entitled ________________________________________________________________.
